Doctor letters, certifications, and forms

California SDI (DE 2501) — State Disability Insurance
California State Disability Insurance physician/practitioner certification (DE 2501 Part B). The clinician must certify the diagnosis, expected duration, and treatment plan under penalty of perjury under California law.
Camp / sports physical clearance letter
Camp or sports physical clearance letter. Required by youth camps, leagues, and schools to confirm fitness to participate (or to document an exemption).
DMV / CDL fitness-to-drive certification
DMV fitness-to-drive or commercial driver license DOT-physical medical letter. Required by state DMVs for civilian licensing and by FMCSA for commercial driving.
FMLA WH-380-E — Federal Family and Medical Leave Act
The federal FMLA WH-380-E certification of a serious health condition. Required by employers when an employee requests FMLA leave for their own condition.
Jury duty medical excusal letter
Medical letter requesting excusal from jury duty for a documented medical condition. Court-specific format requirements; some courts have a separate form to complete.
New Jersey TDB — Temporary Disability Benefits
New Jersey Temporary Disability Benefits medical certification. Provides up to 26 weeks of partial wage replacement for non-work-related illness in NJ.
New York DBL — Disability Benefits Law
New York Disability Benefits Law short-term-disability certification. Required when claiming up to 26 weeks of partial wage replacement for non-work-related illness or injury in New York.
Pre-operative clearance (RCRI + METs)
PCP-to-surgeon pre-operative clearance letter using the Lee Revised Cardiac Risk Index (RCRI) and functional capacity (METs) per ACC/AHA guideline.
Private LTD / STD Attending Physician Statement
Attending Physician Statement (APS) for private long-term disability or short-term disability carriers (Unum, Cigna, Hartford, Aetna, MetLife). The most-litigated form in disability insurance.
Return-to-work letter with restrictions
Return-to-work letter detailing restrictions or full clearance. Creates legally binding employer accommodation obligations under ADA Title I (42 U.S.C. §12112) and state workers' compensation.
School absence + 504/IEP accommodation letter
Medical letter for school absence excusal AND 504/IEP accommodation requests. Required by district 504 / IEP teams to support accommodation plans.
SSDI Medical Opinion — Social Security Disability
Treating-provider medical opinion letter supporting a Social Security Disability claim. The clinician's narrative is often the single most important piece of evidence in an SSA determination.
Travel medication letter (international)
Medical letter for patients traveling internationally with prescription medications, especially controlled substances. Facilitates customs clearance and TSA security screening.
